Alliance Medical Ministry’s Primary Care Clinic employs a paid staff of physicians, physician assistants, nurses, and medical assistants. Board-Certified physicians in Internal Medicine or Family Medicine provide the medical foundation for patients at Alliance. Approximately 250 clinical and non-clinical community volunteers supplement our staff. The Primary Care Clinic is open Monday through Friday from 8:30 am to 4:30 pm, and select Saturdays 9am to 12pm. Patients pay a sliding scale fee based on income, with 98% of visits costing $20 - $25.

Patient eligibility requirements:

  • Resident of Wake County
  • No health insurance
  • No Medicaid
  • No Medicare
  • 18 years or older
  • Must be employeed or live with a family member who is employeed

Patient Application:

All appointments in our primary care clinic must be scheduled. You may download an application from our website and complete it prior to your first appointment. However, please do not mail completed applications. They should be brought with you to your first appointment.

Requirements for first appointment:

  • Arrive 30 minutes before scheduled appointment
  • Must bring photo ID
  • Must bring proof of income. (current year taxes, letter from employer, or two most recent paystubs)
  • Payment is expected at time of service

  • 24-Hour coordination of care

  • Diabetes – diagnosis, eye exams & treatment care
  • Asthma – diagnosis & treatment care
  • Diabetes disease management program
  • Free community-wide screening events
