August AMM Pulse: Community Centered Care

Dear Friend,

At AMM, we provide holistic healthcare that integrates physical, mental, and spiritual well-being. It’s founded on the idea that true health isn't determined by the body alone. This foundational approach in treating the whole person includes 4 elements: 1) the physical, 2) the mental, 3) the emotional, and 4) the social and spiritual aspects of a person's well-being. Celina, our Community Health Worker, commented to me about our patients that I think about a lot – “ Our patients are in survival mode and juggling multiple life crises while trying to maintain their health." Our staff has no “easy“ appointments with patients – no wellness checks – and most patients have trauma in their lives that is hard to imagine. I feel truly grateful to work alongside incredible people whose empathy and compassion exceed anything you could imagine. The stories you might see on TV, read, or hear in church are the stories of AMM patients.

AMM's Community Health Worker (CHW), Celina, goes above and beyond for AMM patients in her role. As a CHW, Celina has delivered groceries and prescriptions to patients, taught patients how to use public transit, and assisted patients in finding much-needed resources.

A CHW is “A frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A community health worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.”

We appreciate the partnership with Southeastern Healthcare of NC in providing AMM with Celina.
Earlier this month, AMM set up a booth at the Tarboro Road Community Center for National Night Out. National Night Out is an opportunity to promote law enforcement, community partnerships, crime prevention, and safety awareness through community fellowship. Members of the community were able to enjoy a night of free activities in association with the Raleigh police department. AMM set up a health trivia Jenga game in addition to handing out information about volunteering and becoming a patient.

Join AMM's farmer, Lauren, in the garden to harvest, weed, water, and more! On August 16th, we kicked back off with AMM's monthly community garden days. One Saturday a month from 9-11 am, AMM invites everyone to volunteer in the garden.
