October Pulse: Innovation & Impact!

Thanks to a generous grant from the William R. Kenan Jr. Charitable Trust, AMM is transforming care for uninsured patients with diabetes—helping prevent serious complications like kidney disease, blindness, and amputations.

Continuous Glucose Monitors (CGMs) are often too costly for our patients, but this grant allows us to pair these powerful devices with hands-on education and real-time support.

With CGMs, our care team can track blood sugar trends between visits, adjust treatment, and coach patients on managing their health day-to-day—empowering them to become partners in their care, not just recipients of treatment. This takes a team—where care doesn’t stop when the clinic visit ends.

We currently have 60 patients enrolled in our Diabetes Care Management Program, and 13 graduates who have lowered their A1C levels by an average of 36%. The chart below highlights their incredible progress.



These changes are significant. And every graduate makes room for a new patient, allowing us to continually extend the reach and impact of the program. We’re thrilled to see this program changing lives—and so grateful to the Kenan Charitable Trust for making it possible to pair cutting-edge tools with compassionate, team-based care.

Dr. Thakur was selected to be a member of the Executive Council of the Wake County Medical Society. 

The Wake County Medical Society was established to enable physicians and other friends of the profession to support projects vitally affecting scientific medicine and public health. Dr. Thakur’s expertise and dedication make her a valuable addition to the Executive Council! 

This past Saturday, AMM teamed up with the Tarboro Rd Community Center to host a "Wellness Walk." Patients and community members gathered around picnic tables at the community center to discuss sleep hygiene with Dr. Thakur. Then, they enjoyed the crisp autumn weather and took a walk around the neighborhood. We plan to host more of these walks in the coming months to keep our community active and informed about their health.

If you have been in the AMM Garden recently, you may have noticed a bright patch of yellow and orange marigolds in the hoop house. These marigolds are more than just pretty flowers; they also serve a purpose for many of our patients.

Every year on November 1st, many families gather together to decorate altars, or ofrendas, to honor and remember their deceased loved ones for Día de Muertos, or Day of the Dead. The scent of the marigolds is said to lead the souls of deceased loved ones to their families, and the bright colors add to the celebration of Día de Muertos.
